In consideration of services provided, upon the hereinafter stated, I hereby forever grant J&K International, including, without limitation, their assignees, licensees, legal representatives, those acting on behalf of J&K International’s permission and authority, the absolute and irrevocable right and permission to copyright and use and reuse images, representations, drawings, photographs, or any other likeness of work performed by J&K International (including without limitation images the bathroom or kitchen of my home, and before and after images of cabinetry installed in my home) without restriction of any kind. The images may be utilized in whole or in part and may be utilized as a composite picture or distorted in form. This grant shall also include the right to change or alter, from time to time, all such photographs, or whether made through any media or process by J&K International for advertising, trade, promotion or any other purpose whatsoever.

I also consent to and grant the right of the use of any printed matter in conjunction with the foregoing. I hereby waive any and all right which I may have to inspect or approve the finished product or products or the advertising copy or printed matter or any other matter that may be used in connection therewith or the use it may be applied. I hereby release, discharge and agree to defend and save harmless, J&K International or those acting on their behalf, from any liability of whatsoever kind, by reason of any distortion, alteration, blurring, optical illusion, changes or use in composite or other form, whether intentional or not, which may occur or be produced in the making of said photographs or any processing thereof, as well as any publication.

Notwithstanding the foregoing, J&K warrants and represents to the homeowner below that any publication of the material, images, drawings and the like to which consent has been given hereunder shall never contain any private, personal identity information about the homeowner, including his or her name, his or her address, the name or address of any family member, or other personal private information of the homeowner.

I further warrant that I am over 18 years of age and have read this release and that terms hereof, prior to its execution, and that I am fully familiar with the contents hereof.
